The Basics

If you are a pre-existing client of Hemporium, please note that you already have access to the services of our GP and we do not require you to fill in a registration form. Please contact the Clinic on 02 4022 8979 to book an appointment.

If you live in the greater Newcastle region and are seeking access to a traditional GP who respects your rights to privacy, informed consent, and who upholds the highest moral and ethical principles, then please register to be added to our books by completing the form below.

We charge an administration fee of $30 per person to register as a client of the Doctor's Surgery. Note that the doctor does not write scripts for Medical Cannabis but can supply a supporting letter if scripts are arranged through us.

If you think you need an ambulance or your injury or illness is critical or life threatening, call triple zero (000) for an ambulance immediately. Urgent after-hours care is also available at the Emergency Department of the John Hunter Hospital or Calvary Mater Hospital in Newcastle.

Instructions

- Please read all questions on the intake form below and answer as best you are able.

- Once you make the payment (at the end of the intake form below) you will receive a payment submission confirmation email.

- Please ensure you check your spam/junk/promotions folder for your confirmation email.

Once we have processed your Registration Form, you will receive a phone call to arrange an appointment with one of our Practitioners. The above process usually takes no more than around 2 working days.

Cancellation Policy

Please note that once you have booked an appointment with us it means that we have reserved time with our Doctor exclusively for you. Because we bulk bill, we do not charge cancellation fees, however the doctor has strict rules regarding late cancellations and no-shows. Please ensure you make any amendments to your appointmentat least 24 hours before it is scheduled to take place. No-shows will not be re-booked and will no longer have access to the GP's services.
